Many people with a disability or illness still want to be independent and with the right support this can be possible. If you need some support to enable you to live in your own home, because of extra needs like a disability or mental illness, ICRIT Supported living has the right support plan for you. You will have carefully planned support. This could be someone helping you out with anything from a few hours a week to 24 hours a day. We will work with you to ensure you have the appropriate support to meet you needs. The right care package is fundamental to the success of our independent living service.
Our Hospital to Housing Support Services is designed to provide support to individuals who have come into hospital and are homeless or at risk of becoming homeless. There is evidence to suggest that being homeless may result in inappropriate admission to hospital, bed blocking, inappropriate discharge and readmissions to hospital. The gap that the Wyresdale housing is attempting to fill is the lack of a coordinated approach to supporting a group of particularly vulnerable individuals by assisting them to find appropriate accommodation and support.
