ICRIT Hospital to Housing Support

Introduction

Our Hospital to Housing Support Services is designed to provide support to individuals who have come into hospital and are homeless or at risk of becoming homeless. There is evidence to suggest that being homeless may result in inappropriate admission to hospital, bed blocking, inappropriate discharge and readmissions to hospital. The gap that the Hospital to Housing Support Services is attempting to fill is the lack of a coordinated approach to supporting a group of particularly vulnerable individuals by assisting them to find appropriate accommodation and support. We assist these individuals in developing care packages aimed at helping them to maintain the accommodation and to meet their health and social care needs.

How it Works

The Hospital to Housing Support Service receive referrals from hospital staff, social workers, and mental health care coordinators. The clients could then be referred to short stay accommodation at our St Georges project before receiving support to secure appropriate long term accommodation. While at St Georges, multi-agency staff are involved in assessing the health and social care needs, which are used in developing comprehensive care packages. The care packages may then be used to support long term accommodation.

Once discharged from hospital, our services will support these individuals in finding long term accommodation. Our services will liaise with Bolton housing and private land lords in an effort to secure appropriate accommodation. The team will work with these individuals in to understand why they ended up homeless in the first place.

Once these reasons are identified which may include among other reasons,

  1. unemployment
  2. poor money management skills
  3. not claiming appropriate benefits
  4. poor social skills
  5. mental illness
  6. substance misuse an action plan will be put in place.

The team will collaboratively develop an action plan which is aimed at helping the individuals to maintain their long term accommodation by learning independent living skills among other skills depending on individual assessed needs. The support action plan may be inform of care packages or referral to other agencies or both.  The care package option is where individuals are supported by our dedicated team in their own homes to meet their assessed needs.  The care package may include support with

  1. medication management
  2. relapse prevention
  3. accessing community
  4. Paying bills and managing money
  5. Help with shopping
  6. Maintaining their environment. 

Following comprehensive assessment of their health and social care needs these individuals may be referred to other agencies such as mental health teams, substance misuse teams depending on assessed needs.
